Before scheduling your one-on-one counseling session, please complete the following forms and ensure you submit all required documents listed below. This will help us make the most of your appointment time and ensure a smooth counseling experience. Your time is valuable, and we want to assist you as efficiently as possible!

Required Documents

Current financial documents are required to provide individualized housing counseling and accurately assess your financial situation. If you have difficulty obtaining a document or are unsure what to provide, please contact us—we are happy to help!

These documents may be uploaded at the end of this form, emailed to jclagett@cchra.com, or dropped off at our main office during normal business hours. Forms must be received before we can schedule your appointment.

Optional Documents:

These documents may be requested by the Housing Counselor after initial review, depending on their relevance to your goals.

Please Note: The Housing Counseling Intake Form should be completed together with a spouse, partner, or any other individuals you plan to purchase a home with, or include in your counseling session.

Once you’ve collected all necessary documents, please begin the form below. This form must be completed in entirety before submitting. You will be unable to save your progress, so please make sure you and any others in your household have necessary information available.

If you’ve recently taken the our First-Time Homebuyer Workshop, please be sure to note this — we will not require you to resubmit some of the information from your original registration form. If any of your personal information has changed since taking the workshop, or it has been more than 90 days since receiving your certificate, please complete the form as a new client.

(Please note that your workshop certificate must state that you took the workshop with the Housing and Redevelopment Authorities of Cumberland Counties.)
